ACTIVE LIVING

When therapy ends, progress shouldn’t have to.

Active Living helps bridge the gap between formal rehabilitation and long-term independence through continued strength, mobility, walking, balance and movement support.

For many people, discharge from therapy does not mean the journey is finished. Active Living provides a next step focused on helping participants continue building confidence, endurance and everyday ability.

THE CONTINUUM THERAPY

ACTIVE LIVING

INDEPENDENCE

Supporting the next stage of progress.

THE GAP

Recovery doesn’t always end when insurance-covered therapy does.

Physical and occupational therapy play an essential role in recovery, rehabilitation and restoring function.

But many people reach the end of their covered therapy visits while still needing support with strength, endurance, walking, balance, mobility and confidence.

That transition can be difficult, especially for older adults, individuals returning home after a hospital or rehabilitation stay, or people who need continued encouragement and structured movement.

Active Living was created to help fill that gap.

THE ACTIVE LIVING BRIDGE

From rehabilitation to everyday life.

Active Living provides a structured transition between formal therapy and independent long-term wellness.

01

Therapy

Clinical rehabilitation provided by licensed healthcare professionals.

Recovery • Treatment • Rehabilitation
03

Greater Independence

Supporting the skills and confidence needed for everyday life.

Function • Confidence • Quality of Life
IMPORTANT

Active Living is not physical therapy.

Active Living does not diagnose, treat injuries or replace licensed physical, occupational or speech therapy. It is a wellness and functional movement program designed to support continued progress after formal rehabilitation or alongside an individual’s broader care plan when appropriate.

Active Living can be appropriate for individuals who are medically stable and ready to continue working on movement, strength and everyday function.

FOR FAMILIES

You may be asking: “What happens next?”

A loved one may have finished rehabilitation, but you can still see areas where they need support.

Active Living may be worth exploring if your loved one:

  • Has become less active after therapy ended
  • Needs encouragement to walk or exercise
  • Has lost strength or endurance
  • Is fearful of falling
  • Needs help maintaining a home exercise routine
  • Wants to stay as independent as possible
